Cognitive Assessment
A cognitive assessment is used to gain information about an individual’s current level of functioning, cognitive abilities, and learning strengths and weaknesses.
The information gathered from assessment can be used to provide specific recommendations to suit an individual’s needs, using their strengths to work on any weaknesses that may be identified. A cognitive assessment can assist in identifying specific learning difficulties, intellectual disability, and giftedness.

The process involves one face-to-face session.
A cognitive assessment is completed in a single face-to-face session which may take up to 2 hours to complete, depending on factors such
as age, ability, and the number of subtests required to
be administered.
For children, parents / caregivers will be asked to wait in the waiting room while the assessment is being completed. Parents are asked to complete a developmental history and forward on the Vineland and teacher questionnaire to their child’s teacher to gather as much information as possible about the child.
After the cognitive assessment, the Psychologist will score all assessments, review developmental history and observations, review past reports, and liaise with other therapists (if necessary).
A comprehensive report including developmental history, observations, assessment results, and recommendations is developed. A feedback session is scheduled which is completed over the phone. The Psychologist will provide a draft report for the feedback session and will go through the outcome of the assessments, recommendations and answer any questions.
The Psychologist aims to complete the draft report and feedback session within 4-6 weeks post-assessment.
The tools we use for the assessment may include:
- Wechshler Preschool and Primary Scale of Intelligence - Fourth Edition (WPPSI-IV) [for ages 2 years, 6 months to 7 years, 7 months]
- Wechsler Intelligence Scales for Children – Fifth Edition (WISC-V) [for ages 6 years to 16 years, 11 months]
- Wechsler Adult Intelligence Scale – Fourth Edition: Australia and New Zealand (WAIS-IV A&NZ) [for ages 16 years to 90 years, 11 months]
- Vineland Adaptive Behaviour Scales - Third Edition (Vineland-3); Comprehensive Teacher form
